Patriot Service Dogs is a 501(c)(3) organization working to provide quality service dogs to veterans in need. We invite volunteers over the age of 21 and of any race or creed or sexual orientation to engage with us and provide
vital skills to our dogs in training.
After we receive your application, we will contact you and arrange for an interview in person or by phone with our volunteer coordinator. All information on this form will be kept confidential and will help us find the perfect volunteer project for you.
Are you physically able to manage a young, large breed dog?
All of our dogs are young, large breed dogs. Volunteers help train them when they are still puppies in training, so they will not be perfect service dogs. Please be realistic about the dogs and your own abilities.

Volunteering with Patriot Service Dogs is a wonderful activity for the whole family. We encourage both
families and individuals to fill out an application. We welcome all volunteers regardless of age, race, sexual orientation, or religious affiliation.
Please list the adults in your home (over the age of 18 years old) and their relationship to you. Please list all children (17 and under) that reside in your home and their age. Please list all of the animals in your home. Include the type and age of the animals. Does your entire household support this project? Please describe your home (size, fenced yard, flooring types, etc.). Experience:
Patriot Service Dogs is interested in your history with dogs and the training they may have received. No experience is required, just a willingness to learn! If you do have previous dog training experience, we will expect you to use our methods, even if they differ from what you've done previously.

IF WE FEEL IT NECESSARY, DO WE HAVE YOUR PERMISSION TO CONTACT THEM TO CHECK ON YOUR PREVIOUS CARE OF ANIMALS? Being a puppy raiser/ Weekend raiser can be an exciting and rewarding experience. We do take this volunteer position very seriously so please consider the following before signing below.
EXPECTATIONS OF A PUPPY RAISER
1. Care for the needs of an 8-week-old puppy, which will need attention every two hours during the day and
possibly during the night for the first month.
2. Crate training/housebreaking
3. Spend at least 30 minutes a day doing obedience training, working on at least 4-6 or more commands each
4. Spend at least 3 hours a week socializing the puppy in public places. This includes having the puppy
accompany you to shopping excursions, doctors’ appointments (when appropriate), religious services, and

provide all kennels, bowls and necessary supplies.

9. Released service puppies/career change puppies may not be offered to the puppy trainer/raiser. Our goal is to
find a working position for all of our puppies whenever possible.
I am willing and able to complete the expectations of a PSD puppy raiser. I understand that any and all animals in my care belong to Patriot Service Dogs, Inc. and must be returned to PSD immediately upon request. I agree to return the dog immediately when requested. Consent, Release and Hold Harmless Agreement
In consideration of the services of Patriot Service Dogs, Inc., its employees, directors, agents, officers,
volunteers, participants, and all other persons or entities acting in any capacity on their behalf, and for other
good and valuable consideration, I hereby agree to release and discharge them from liability arising from
negligence, on behalf of myself, my spouse, my children, my parents, my heirs, assigns, personal representative
and estate as follows:
1. I acknowledge that animal operations entail known and unanticipated risks that could result in physical or
emotional injury, paralysis, death to myself and other persons, and also property damage. Risks include, among
other things: bites, scratches, torn skin, bruises, and damaged clothing or other property. I understand that such
risks simply cannot be eliminated without jeopardizing the essential qualities of the activity.
2. I expressly agree and promise to accept and assume all risks existing in this activity or as a result of contact
with an animal. My participation is purely voluntary, and I elect to participate despite the risks.
3. I hereby voluntarily release, forever discharge, and agree to indemnify and hold harmless Patriot Service
Dogs, Inc. from any and all claims, demands, or causes of action which are in any way connected with my
participation as a volunteer, my use of their equipment or facilities, my adopting or fostering an animal arising
from negligence. This release does not apply to claims arising from gross negligence or intentional conduct.
Should Patriot Service Dogs, Inc. or anyone acting on their behalf, be required to incur attorney's fees and costs
to enforce this agreement, I agree to indemnify and hold them harmless for all such fees and costs.
4. I certify that I have adequate insurance to cover any injury or damage I may cause or suffer while
participating, or else I agree to bear the costs of such injury or damage to myself.
5. In the event that I file a lawsuit, I agree to do so solely in the state of Florida, and further agree that the
substantive law of Florida shall apply in this action.
6. I agree that if any portion of this agreement is found to be void or unenforceable, the remaining portion shall
remain in full force and effect.
By signing this document, I agree that if anyone is hurt or property is damaged during my participation in this
activity, I may be found by a court of law to have waived my right to maintain a lawsuit against the parties being
released based on any claim for negligence from which I have released them herein. I have had sufficient time to
read this entire document, understand its contents, and I agree to be bound by its terms.
I certify that the statements made in this volunteer application are true and have been given voluntarily. I
understand that this information will be held in the strictest confidence, and I release Patriot Service Dogs, Inc.
from any liability whatsoever for supplying such information. I also understand that I will not be paid for my
services as a volunteer I agree to the policy. Photo Release
I understand that any photo taken, given or electronically mailed to Patriot Service Dogs can & may be used in
any publications or the website and all will become the property of Patriot Service Dogs and I agree there will be
no compensation for any usage of said photos in publications or on the website. I also give permission to use any
photos submitted or taken of myself or family may also be used in publications or on the website. I agree to the photo release I agree that the information given in this application is correct to the best of my knowledge. I agree.
